Takeaways From Virtual NDR & ADC Combo Data to Be at ASCO 2024

Last week we hosted a series of investor meetings with the G1 Therapeutics (GTHX)management team. Additionally, ASCO 2024 abstract titles dropped this morning unveilingthat GTHX will have a pos ter with updated data from the Ph2 trial of trilaciclib in combination with the ADC sacituzumab govitecan (Todelvy) in patients with 2/3L mTNBCstudy at the conference.

🌑Investors were focused on cash runway and pathway to becoming cash flow positive. Management guided for cash runway into 2025, which gets them through two high impact readouts: (1) the Ph3 PRESERVE-2 final OS analysis in 3Q24, and (2) Ph2 trilaciclib + Todelvycombo safety and OS update at ASCO 2024 (June 2). This cash runway includes launch/filing costs in TNBC. OpEx was reduced significantly in 2023 and the expectation is that it will continue to come down in 2024 — awaiting guidance on next week’s earnings domains.available.forsale/c... believes they have the potential to become cash flow positive in 2025.

We currently model (only SCLC revenue) GTHX generating positive net income in 2026, but see potential for 2H25 revenue in TNBC (if PRESERVE-2 hits on OS) to move this forward.

🌑FY24 COSELA guidance of $60-70M — some thought this was conservative given strong growth seen in 2H23. Penetration was strong in 2023 despite headwinds caused by the platinum chemo shortage; management doesn’t expect this to be an issue in 2024. Though Management continues to stress that duration of therapy in ES-SCLC is pretty short (4 chemocycles at most), thus every quarter there is almost 100% patient turnover. Time on therapy is much longer in TNBC which is another reason why approval in this indication will be a largevalue driver for GTHX.

🌑If the Ph3 1L TNBC PRESERVE-2 study hits on OS, GTHX will file an sNDA in TNBC as soon as possible. We believe there is a possibility that they could get approval for trilaciclib use in combination with gem/carbo beyond 1L TNBC off of this data set. GTHX retains worldwiderights (beyond Greater China) for trilaciclib so do have the ability to seek a partner for expansion in ex-U.S. markets.

🌑Next steps for the ADC combo program. The current single arm, Ph2 trial in 2/3L mTNBCis a 30 patient signal finding study that will be reading out updated safety and efficacy data at ASCO in June 2024. Recall the company disclosed promising preliminary OS data at JPMHC (see our note here), well exceeding the internal bar previously stated by management of improving on ASCENT mOS by ~2-3 months. If strength of updated data is consistent with the preliminary readout, GTHX will likely partner with a company that has a TROP2 ADC in-house(e.g., Gilead, Daiichi Sankyo) and initiate a randomized, Ph3 trial. There was some investor concern on what timing for such a trial would look like given OS endpoint takes a while to reach. We believe GTHX management/a partner could potentially discuss using an earlier myeloprotection endpoint with regulators.
