According to our findings, the four sets of diagnostic
criteria for AS are discrepant to a large extent, the overlap
being in the worst case only 39%. One factor which heavily
increases the differences between these sets of criteria is
the divergent number of items which should be fulfilled. In
practice, replacing some of the obligatory items with
alternative noncompulsory ones would apparently reduce
the current discrepancies between these sets of criteria.
Another explanation for the large discrepancies lies in
the fact that the degree of severity and difficulty of the
symptoms is not taken into account at all in the criteria for
AS. Classification is prone to rater-induced variation, as
none of the items in the diagnostic sets of criteria are
defined in detail. As a result, interpretation is also easily
influenced by sociocultural variation. Strict guidelines for
interpretation would reduce this variation. The language
delay as an exclusion criterion in ICD-10 and DSM-IV
should be applied in more general terms or removed
because, at least in our study, most of the AS children had
difficulties and peculiarities that could be falsely interpreted as a developmental delay. Another potential
problem may lie in the ICD-10 and DSM-IV not sufficiently taking into account certain symptoms, such as
nonverbal difficulties and motor clumsiness, which other
diagnostic sets of criteria for AS rate as important.
It should be noted, that some of the differences between
the four sets of diagnostic criteria found here might stem
from methodological weaknesses. First, our study is limited
by its small sample size. Our results must, therefore, be
interpreted with caution. Kappa values are likely to be
more stable in larger samples. Another potential source of
bias lies in the sample being a clinical one. The children
were originally referred for further examinations due to
clinical problems, and therefore, the proportion of more
severely affected patients is likely to be higher than in
average AS populations. Finally we did not consider
comorbidity even though co-morbid features might have an
effect on whether a certain criterion is fulfilled or not. For
example, as withdrawal is prominent in the set of criteria
by Szatmari et al. (1989), a child expressing such behavior
due to depression is more likely to meet this criteria.
Likewise, since obsessive-compulsive features are prominent in the criteria of Gillberg and Gillberg (1989), a child
expressing such features more probably would meet this
criteria.
In conclusion, the diagnostic delineation of AS is
incomplete reflected in highly diverging prevalence rates.
The diagnostic criteria for AS need revision and international consensus. More studies addressing the validity of
the diagnostic concept are clearly needed. Larger clinical
studies and normative or case-control studies would be
particularly informative. Future research should aim at
comparing the AS criteria with criteria for autism and other
neuropsychiatric disorders and the most important differences between AS and high functioning autism should be
identified, if any exist. Finally, comparisons of the phenotype of AS in boys and girls and the potential influence
of age and developmental level would be an interesting
area of research
